 - Practice nurse visits for influenza vaccination, care plan preparation and long term condition reviews for housebound elderly patients

Orchard Surgery offers:
- Effective recall for Chronic Disease Registers
 - 20-30 minute appointments as appropriate for reviews of conditions
 - LTC trained nurses – Stroke, CHD, Diabetes, COPD, Dementia
 - NHS Health Checks (by invitation) – early diagnosis
 - Insulin initiation
 - Community Diabetic nurse
 - Identification of Carers and offer of support where applicable
