Orchard Surgery offers:
- Effective recall for Chronic Disease Registers
- 20-30 minute appointments as appropriate for reviews of conditions
- LTC trained nurses – Stroke, CHD, Diabetes, COPD, Dementia
- NHS Health Checks (by invitation) – early diagnosis
- Insulin initiation
- Community Diabetic nurse
- Identification of Carers and offer of support where applicable

- Annual review in patient’s home if needed
- Robust recall system
- GP or self referral to counselling/talking therapies (Time to Talk)
